By getting to know you and what is important to you we can design a programme that supports your goals and keeps you independent. This program could involve support with attending appointments, connecting you to the community, helping you around the home or helping you get to and from activities to support you to stay well.

We also have services that provide that extra peace of mind. Our remote home monitoring solution allows you to live independently in your own home, knowing that help is there if you need it through the push of a button.

Home and Community Care Services are accessible to people under the Commonwealth Home Support Programme (CHSP) and Home Care Packages.

If you have been assessed as eligible for CHSP or a Home Care Package you can request for your referral to be sent to McLean Care; when we receive your referral our staff will contact you to work out how we can best support you to plan for and achieve your goals.

All the services offered by McLean Care are available under a fee for service model; meaning you can have any of the services you want and need. Contact our Home and Community Care office on 1300 791 660 to have a personalised quote developed for you.
