Short-term restorative care

Recovering after an illness, injury or hospital stay can be challenging, especially when you want to stay in the comfort of your own home.  The Short-term Restorative Care pathway is part of the new Support at Home program and provides tailored support to help you regain your strength, confidence and independence.

At McLean Care, our qualified clinicians and care partners will work closely with you to create a personalised recovery plan, focused on your goals and what matters most to you. With the right support, you can get back to doing the things you love, safely and comfortably at home.

Recovering at home can be easier and more effective with the right support. Restorative care focuses on helping you regain strength, confidence, and independence so you can return to your normal routines and lifestyle.  The program aims to:

 

  • Improve your mobility, strength and confidence
  • Reduce your risk of returning to hospital or entering residential care early
  • Achieve your recovery goals with a plan tailored to your needs
  • Receive coordinated support from registered nurses and allied health professionals

How it works

We take a personalised, goal-based approach that starts with understanding your needs and ends with you feeling confident and capable again. Our restorative care partners coordinate your care every step of the way.

Our restorative care partners are qualified clinicians who will:

  • Work with you to set achievable recovery goals

  • Coordinate the right mix of services to support your independence

  • Monitor your progress and adjust your plan as needed

  • Link you with specialist services if required

Eligible older Australians can receive up to 12 weeks of short-term, goal-oriented care, based on a clinical assessment and designed to restore function, confidence and wellbeing.

What services may be included?

Your recovery plan is designed around your goals and lifestyle. Depending on your needs, we’ll bring together a range of services and supports to help you rebuild your skills, health, and independence at home.

Depending on your needs and recovery goals, your care plan might include:

  • Allied health therapies (such as physiotherapy or occupational therapy)

  • Nursing support and wound care

  • Personal care assistance

  • Support to rebuild daily living skills and confidence

  • Help with nutrition, mobility aids or home safety

Every plan is unique - designed to help you recover well, stay independent and feel confident in your own home.
