What are the benefits of Transition Care?
Transition Care helps you recover after a hospital stay. It provides short-term specialised care and support in your home to help you regain your functional independence and confidence sooner.

Who can provide these services?
McLean Care works with Darling Downs Hospital and Health Service to provide care within the Transition Care Programme model. Your discharge planner will review your eligibility for the program and manage the referral to ACAT while you are in hospital. McLean Care will commence support and services upon your discharge home. Our team will be working along side you as you regain your confidence and functional independence.

What can a client expect from transition care services?
Once discharged home from hospital a client will receive a visit in their home from one of our friendly nursing staff.

We will work with the clients’ TCP Case Manager to meet the needs and goals of the clients’ written care plan. The client may wish to have a family member or friend present during this initial visit.

We understand that each clients’ care plan is different and may include various services such as assistance to shower or wound care. We will adapt our approach for each client we meet.
